You can print and fill out the appropriate intake form(s) so that you can bring them with you at your appointment! Be sure to review any contraindications on the form and if you must cancel, please let us know 24 hours in advance to avoid a cancellation fee.

Intake forms are also available when you arrive. Please arrive 15 minutes prior to your appointment time to review and complete your form(s) so that you do not lose time in your session.
